from your local hospital, or you can download a
form specific to your state from the websites of
the U.S. Living Will Registry and the Partnership
for Caring. You also can create your living will
using an online tool or software. Additionally,
many of these tools help you create a durable
medical power of attorney. Here are a few options:

The site charges $6.99 to walk you through the process. You can have
your finished living will sent to you via email as a PDF or Word file.

Legalzoom.com
For $39, the site helps you create the living will, has a legal professional
review it, prints it on acid-free bond paper, offers health care power of
attorney provisions and provides notifications of relevant changes in the
law.

Wills-online.com
A fee of $11.95 covers optional health care power of attorney provisions
and the creation of the living will, which you can save to your computer as a
Word or PDF document. Additional fees apply for printing and legal
assistance.

Kiplinger's WILLPower
The software helps you create a state-specific living will, as well as property
wills, trusts and other documents. Download or purchase it on CD for
$29.95.
